Thanks for your feedback. I agree there is always a chance that an important element will fall through the cracks. It's critical that the process and the documentation be revisited regularly and reviewed with stakeholders to ensure it's performing as expected.

I think Mike's approach to the documentation leave to room for any thing to fall between cracks which is usually the case with some process mapping , when it happen it my remain for long time and people may monkey-do it for along time before somebody come and say umm why do we do it this way . and I thing the most important is the segregation between procedures and Process maps , which many got wrong .

Following many years of experience in IT Project Management I was considering writing a very similar article taking into account the lack of competent Project Sponsorship and requirements specifications. I was also considering the fact that Agile is now seen as the solution to everything following my recent experience of working in an international infrastructure project that the company wanted to do in Agile Methodology. My big concern in this area is the impression that one methodology fits all projects. When in reality most companies don't follow their own methodologies when it comes to the crunch.
I enjoyed this essay and will continue to work on my view of this subject
